Description

Implementation of digital health tools to improve COPD clinical guideline adherence and direct referrals from emergency department ED to outpatient clinics and primary care using implementation science and mitigate practice gap to improve discharge process and referrals. Electronic proformas have the potential to facilitate direct referrals from the ED utilising integrated electronic medical records (IeMR), leading to reduced hospital admissions, reduced length of hospital stays, holistic care, improved health care and quality of life and improved interdisciplinary guideline adherence.
